Instructions on Visa Application

Step 1: If you have not downloaded the visa application form, please download it here:

Step 2: Filling in the form

Question 1:	Full name Please give your full name in BLOCK LETTERS Example: NILS STEFAN HELLMER
Question 2:	Date of birth Please give your date of birth in Day/Month/Year format. Example: 27 JUNE 1974 or 27/06/1974
Question 3:	Place of birth Please state the name of the city and country where you were born. Example: STOCKHOLM-SWEDEN

Step 3: Photograph

Please attach 01 passport size, color or back and white photograph on to the application form.

Step 4: Before you send the application

Please make sure that you have included:

• your passport;
• visa application form;
• visa fee in cash
• a self-addressed return envelop. (you don't need a return envelop if you apply at the Embassy )

For maximum safety, please use registered/recommended mail service to send your visa application.
